We recognize that current and former clients, as well as others to whom we mail or otherwise provide information or services, have entrusted us with personal information. We recognize our obligation to keep information about you secure and confidential. This notice describes how we handle your financial information that we have collected. It is important for you to know that we do not sell your information to anyone. We restrict access to nonpublic personal information about you to employees who need to know that information in order to provide advice, products or services to you. We also maintain physical, electronic and procedural safeguards to protect your nonpublic personal information.
Mazza Financial Group, Inc. collects nonpublic personal information about you from applications, forms and other written material, oral communications and electronic mail (email), which we receive from you.
Some employees may be associated with nonaffiliated businesses. They may act as Registered Representatives of Pacific West Securities, Inc. or Registered Investment Advisory Representatives of Pacific West Financial Consultants, Inc. All of the information that Mazza Financial Group, Inc. collects will necessarily be disclosed to them in those capacities. Information they collect from you in those other nonaffiliated capacities will also necessarily be disclosed to Mazza Financial Group, Inc. In both cases, however, they will not disclose the information if you notify us that you do not want the information used in any capacity other than the one in which it was provided to us.
If you wish to opt out of such disclosure, please call (800) 762-3260. Understand that by opting out, it will prevent the disclosure of your information between employees of Mazza Financial Group, Inc. and the use of that information in the context of their associations with nonaffiliated businesses. For example, if you own a term life insurance policy, we would not be able to discuss investments or investment advisory services with you without first getting your permission.
At such time as Mazza Financial Group, Inc. and an employee terminate their relationship with one another, both parties may retain information that has been disclosed by one to the other, if they have so agreed as a part of their employment agreement.
We may also provide nonpublic personal information about you to nonaffiliated third parties as permitted by law. An example of this would be the sharing of your information with the Office of the Insurance Commissioner.
